Help me keep my dog

Logan was diagnosed diabetic this year.  He needs insulin injections twice a day and special prescription food.   I have reached the point where I am no longer able to provide these for him without assistance.  I have enough supplies to last for about the next month.  After that, he'll either be without his supplies or I'll have to surrender him to a shelter. =(
