21st Birthday - Please Help Me See!

Okay, so to start off my birthday is in roughly a month and I'm going to be 21!!!! Now, as excited as I am, all I really want for this big day is not to drink, party, or even receive any gifts (well, sort of). All I truly want for my birthday is to be able to see.
I absolutely never do anything for my birthday, or ask for anything at that. I actually have worked every birthday since I've started working at 16. Since I'm turning 21 this year the typical thing to do is blow a bunch of cash and time on myself and just let go. Problem is that I have trouble doing that, especially when I feel like I miss so much. So I've decided that I'm not going to do that. All I want more than anything for my birthday or just in general is laser eye surgery so that I can see. My eyes are pretty bad off (just ask my doctor) and even for things less than a foot away I need to squint to see clearly. How blind am I? Well, legally, I'm not allowed to drive at night without wearing glasses/contacts.
But I'm sick of all that. I've worn glasses since I was 10 and it served as well as a bandage on a broken leg - my eyes just continued to worsen. Also, with all the sports I did it was impossible to have them on and do anything without them falling off and to not be able to see flying objects hurtling towards you can get a little dangerous, as you can imagine. Then I moved up in the world and was introduced to contacts at about 13. It was great! Except that my astigmatism is so intense that they were as thick as stationary and would cause extraordinary amount of discomfort. If they ever got stuck in my eye it would hurt so immensely that I would punch walls and thrash from the pain because they were so thick it was near impossible to pull them out! And if that weren't bad enough, on my 18th birthday a contact shattered in my eye and I had to wear an eyepatch for a week. Happy birthday to me.
Another reason I want to get this done is that although I work two jobs and work between both of them 11-12 shifts a week everyday unless I absolutely need a day off, I can't afford new glasses/doctor visits because I have no insurance. My last pair cost my mom $600 and that was a few years back. Now they fall off my face and are scratched to hell but they're all I have. I can't imagine what they would possibly cost now..
It truly sucks to be blind with an intense astigmatism (not to mention both my eyes have different visions). All I want to do is be able to see. That's it. It's going to cost around $4000+ and I can't afford that. As a favor to every and any of my true friends, or even random stranger that come across this, all I want for my birthday this year is a small donation to help me pay for my laser eye surgery. I couldn't ask for anything more or anything ever again. It is my one and only birthday wish.
