Help with stage5 kidney failure costs

I am currently battling stage 5 kidney failure. I have had my fistula(dialysis port) put in and will start dialysis soon.Im waiting on my surgery site to heal, which is infected. I am in the process of getting on the list for a pancreas / kidney transplant. I need help paying for transportation to Birmingham from Selma. Attending these appointments in Birmingham is mandatory to get on the transplant list. I also have type 1 diabetes. Insulin shots does not regulate my blood sugar. I require an insulin pump. Unfortunately my insurance will not cover the costs of the supplies for my pump. I also have hypothyroidism and neuropathy. I need help buying the medicine I need. I have been unable to work since December 2014. My wife of 12 years is unable to work. My health is unstable and she helps take care of me. We have 3 kids we are trying to take care of. Ages 12, 9, and 7. We need help buying school supplies and clothes. I also need help in paying our bills. Any help would be a blessing.
