Trying to get back on my feet

My pain has been great for years and landed me in the hospital multiple times. I have continued to try to maintain and work throughout all of the pain. I recently was able to file for FMLA to have the hysterectomy that was so desperately needed.  The tumors ended up being bigger than they thought. The biggest being the one in the photograph.  In this time for recovery I was supposed to be granted the short term disability to be able to at least try to keep up on bills while I was out recovering. I did all I needed to do and the company is still requesting another form in which i have called and emailed the responsible party regarding. I cannot get a call back or email or any answer as to what I still need to do.  I need a little help getting through this time and keeping my head above water until they get this figured out.  I hate asking but I am at a loss. I greatly appreciate assistance and will surely pay it forward.
