Results from my appointment with my new surgeon.

I independently went over AP, lateral, flexion-extension radiographs of the cervical spine obtained in the office today. There is dominant findings at C5-C7 where there is advanced collapse of the disc space with some posterior osteophytes as well as some reversal of the cervical lordosis in the setting of the significant degenerative pathology. There is a mild listhesis at C4-C5. Findings are all chronic.

I independently went over his previous MRI. This MRI is consistent with significant findings from C4-C7 with mostly disc osteophyte complexes at C5-C6 and C6-C7 resulting in some significant stenosis at both levels. There is no focal signs of myelomalacia.

Today we reviewed clinical findings and treatment options with the patient. Clinically, he presents with clinical symptoms of cervical radiculopathy in the setting of advanced spondylitic changes particularly from C5-C7. I do agree that his MRI is impressive and I do agree that his clinical symptoms of cervical radiculopathy is consistent with his imaging.

I went over options for treatment including surgical versus nonsurgical options. Although surgery is reasonable in his case he is still an active smoker and if he is able to demonstrate that he can be nicotine free for at least 6 weeks prior to the surgical intervention then I might possibly be able to consider surgical intervention with his case which would involve a likely anterior cervical decompression and fusion. Prior to this he needs to demonstrate smoking cessation and medical optimization. He is aware of this. I briefly went over the surgical intervention and he understands the criteria for that needs to be met prior to surgery.

Regarding his disability status I do not plan to comment on his overall disability status at this is a second opinion to see me. His disability rating would be as previously provided by his other treating physicians following this injury from 5 years ago. He can also be referred to our occupational medicine department for any further questions regarding disability. For now he can see me back if he is able to stop smoking to discuss possible surgical options. All questions were answered. Patient voiced understanding and was agreeable with the current instructions and plan.

In September by the 5th I have $85 due for my storage unit. Then between September 12-17 iI have my cell phone bill $50 and also my PO box due adding another $63. I'm so over trying to scape together any kind of money just to slide by. Any help even the smallest amount will help me greater than you can imagine right now.

Surgeon last Thursday told me I need to quit smoking for 6 weeks to get him to consider doing my surgery. I'm down to my last remaining cigarettes and I'm switching to nicotine pouches for the rest of the week. Next week I'll start my 1st week of no nicotine.

We'll had my workers compensation hearing and it's at yet another stand still again. Supposedly there where work searches missing from my file even though I had copies of them on me. Some how they never made it to the board for the judge to see. So it's on hold until end of September or into October. On top of that the MRI place never mailed out a copy of my MRI which I talked to them last Friday and they said they'd mail it out.

I'm literally on my ropes end and am trying to push through it, but there's only so much one person can handle.

Haven't had kind of money in over 2 years now with no end in site.

Finally received my MRI results in my workers compensation file on-line last night.

C4-C5 disc herniation, Bone Spur 2mm

C5-C6 disc herniation and moderate to severe degree of bilaterial foraminal stenosis greater on the right. Bone spur 3mm

And C6-C7 slight retrolisthesis of C6 over C7 and disc herniation and also moderate to severe degree of bilateral foraminal stenosis greater on the right. Bone spur 4mm

Going to Kenmore today to have a open MRI done on my neck to figure what's going on since it's been worse the last 3-5 months. Hoping the MRI shows what's wrong and what can be done to relieve the pain and stiffness. Don't get some doctor's at the being of this which was in October 2021. They told me if it traveled to my extremities it was more serious. It was in my shoulders, arms, and hands all the way back in 2021 and it was basically blown off.

Found a place to stay for awhile with my

friends until we can find something permant together. Last donation is being used to pay my locker/storage unit rent for August.

No matter the amount I'm appreciative of anything sent my way, and I greatly appreciate it a lot.

Was going decent for awhile. Now the friends I'm living with are having problems. Last night at around 7pm we received a 72 hour notice from Sheriff's to vacate the premises so we have until 7pm Thursday more than likely. Looking for apartments in erie county preferably Akron, Alden, williamsville, Clarence.

Dealing with this now on top of everything else. Have a compensation hearing August 19th and a surgeon's appointment on August 30th. And I'm working on getting a MRI scheduled that got approved from insurance carrier for my neck.

Again I'm asking everyone for any sort of financial help no matter how small anything would help at this point. I also have $85 due for my storage unit with my belongings due by August 5th.

Starting a appeal on my Social Security Disability, but that will possibly take 6-12 months is what my lawyer told me. I applied for Unemployment VR access for retraining, and haven't heard anything back since i sent paperwork back and it's been 3-4 months. As for my workers compensation case is in a holding pattern right now. With really no end or settlement in sight. Lawyer is working on trying to restart my payments, but there only going to be $150 a week. The only thing that might help me if he gets me the 6 month back pay for being attached to the work force by doing weekly job searches with my restrictions. No one is going to give me a job starting out on light duty with my restrictions.

Any help would be greatly appreciated on my behalf

Still waiting on a hearing date for my workers compensation case. There's a possibility I may receive 6 months back pay, and my lawyer is working on a settlement but that could be quite a while away still. My social security disability case was denied, but I'm appealing it again.

Found out today that my lawyer never requested a hearing for my workers compensation case until today. It's been 5 months with nothing happening. Could of probably at least had some workers compensation checks already after all since it would of been re-enstated by now. And found out he hasn't even started on my social security disability appeal as of yet. I'm telling you I'm barely holding it together right now.

The new picture is of a trailer in Akron that's still up for sale, but I don't have any money coming in or any of my cases moving into settling or back pay yet.

Really can't catch a break in anything for the life of me. I did get a new hearing date for my social security disability case in early April which is still over a month away, and even I get approved it'll be months more than likely before I see anything. As for my compensation case it's in a current standstill at the moment.

I have so much stuff do this coming month all around the same time. My $35 for our washer rental is due, $85 is due for my storage unit with the stuff from my trailer in it. My PO box is due $77 up from the $65 which it was when I got it in October which is for 3 months for the smallest box they have. I need a break in either case leading towards any kind of income that I can survive on for right now.

If anyone can donate to me now matter the amount it would be greatly appreciated on my behalf. Never had to basically beg like this before in my whole entire life before, but it's all I can do currently to try to get by until anything happens in either case.

Had a phone interview yesterday for unemployment VR access and possible retraining, and help with redoing my resume and such. And found out today that my social security disability case hearing is reset for April. Everything seems so incredibly slow on either case seems to take forever to get anything done at all. It's incredibly since it was instant when workers compensation cut my payments back in May 2024, but to get them started again takes years.

Little bit of a update on what's been going on. Still can't seem to catch a break on either case. There both in a stand still and there's no movement in either one currently. The ssd case still haven't a new hearing date since the other one at beginning of December. In addition, the workers compensation case is in a holding pattern. Waiting on unemployment to get back a hold with me for VR meeting/classes after I sent paperwork back to them at the beginning of December. Which is something I need to do for workers compensation. In May it'll be 2 full years without any kind of true income/money. Lost my trailer at beginning of October of last year and been living with some friends which has been a god send to say the least. Just want something to happen so I can start to rebuild my so called life right now.

Could still use any sort of help no matter how small the amount is to help me get through this all. Need to pay my storage unit fee for next month and my PO box is due soon and the post office increased the price up to 77.00 for 3 months.

Can't seem to catch a break for the life of me no matter what I do on my end. Lost my mobile home at the beginning of October, and since then I can't catch a break on either of my cases. To get any kind of income flowing to help me get forward rebuilding my life again.

Made it to my lawyers Thursday for my SSI hearing and long story short we missed even though I was there at 9:50 and hearing wasn't until 10:30. So I don't know how long it's going to be to have it rescheduled it could be couple weeks or a couple months. I can't seem to catch a break for the life of me. And what happened has nothing to me not showing up on time.

Just looking for help to get me through until late February into March. Looking for between $400-$600 to push me to the finish line. Need to pay my storage unit each month which has all my saved belongings in it. Also need to pay for my USPS PO box I opened in October in Batavia until I figure out what I'm going to do when any kind of settlement and back pay possibly comes in. Keeping my options open until I figure things out.

My Social Security Disability hearing is for December 4th by phone. As far as my New York State Workers Compensation permancy it's on hold yet again. Judge came out with only a 30% loss from my neck and back. My lawyer is appealing the ruling. So I'm not looking at getting any sort of money from either until maybe February or March.

Another 2 mobile homes went up for sale a 2,000 one and a 4,000 one, but both are investor/contractor specials and not being offered to the general public for sale currently. Wouldn't mind the 4,000 one which looks to be a double wide possibly it doesn't look to bad. Might have a possible go around on the trailer parks investor labeling, because I have a friend that is a contractor.

The park I'm in now only has two trailers up for sale currently. One is $7,000 and is a contractor special so I can't even look at it. The other one is $34,000 and it's a 1977 has a open car port and also a shed, and description says all appliances are included. Even with all that's it's not worth what there asking and it's Hunt selling it. They originally had it up for 60,000 so they have dropped price quite a bit.

Scheduled a walkthrough with the trailer park for today and they called me yesterday and cancelled. Supposedly they already sold (under contract) the trailer even though it's still up for sale on multiple sites. So I guess it's time to look for more mobile homes in the area.

Waiting on trailer park sales team to e-mail me with interior pictures of the trailer. I'd like to possibly schedule a walk through of the property so i can see the condition of the inside of the trailer to see what kind of work needs to be done to it, and just to see the layout of everything like the 2 rooms and the 2 full bathrooms. I already can see that the kitchen, dining room, and living room are in the middle of the trailer. Apparently they have been away on some sort of sales training.

I've looked over the outside of the trailer twice yesterday and today again. Definitely needs a little bit a work on the outside. There's a window on the left side of the trailer that needs the wood framing redone or just taken out and replaced with a better window and framing. In addition, the wood framing around the garage door definitely needs to be replaced. E-mailed whoever has it up for interior pictures last night no response yet. Going to request a walk through for maybe Thursday or Friday this week just to look at the interior.

Spotted a 1984 trailer that the park or owners have up for sale for $3,100 only and lot rent is like $680 a month. The trailer park is kept up pretty nice, and they seem a lot more active than my old trailer park owners.

Moved into my friends place last Thursday and I'm all settled in as of today 9/6. Could of grabbed more stuff from my trailer since the Sheriff's and the trailer park showed up this morning (found out from one of my neighbors). Oh well I'm all passed that right now. Just trying to figure out my future plans.

Just need something to happen in my favor either my workers compensation case or social security disability claim. Probably going to be my workers compensation first since social security disability is only in step 3 of 5 so far.

Ended up filling the 2nd trailer up yesterday plus filled a small pickup also. Moved most everything to my storage unit, and have it pretty we'll packed up with my stuff. And I've moved in with one of my close friends last night. I've got a mid size bedroom which I'm totally happy with for right now instead of a county hotel/motel room.

Going to need to get a PO box to send my mail to for the time being. Priced them and there $60 for a small box for 3 months which it's fine, but it automatically renews itself which is sort of the bad part.

Have the 1 of 2 trailer loads ready to take up to my storage unit. Then i plan on filling it a second time tonight and Wednesday to take up on Thursday.

Bought 4 more 27 gallon stackable totes from Walmart today. Plan on filling them Monday getting ready to move them on my friends trailer. I plan on possibly moving them on Tuesday and refilling trailer Tuesday and Wednesday to possible move 2nd load on Thursday sometime. So slowly but surely things are moving along.

I've come to the realization that I'm going to loose my trailer which in a way I'm good with it as weird as that sounds.

So I've been using the donations wisely as I can. Bought some totes to pack the belongings I plan on keeping and I plan on grabbing a couple more (Wal-Mart has 27 gallon stackable totes for $9.97). And I rented a 6x10 storage unit for October and I've had a friend drop off his open ended trailer that I've started packing.

This is actually me trying to get help right now.

Trailer park e-mailed me back and is requesting the full amount to cancel the lock out which is a little over $9,000 right now.

Moved into my mobile home with my dad in 2011. Been by myself since 2017 or 2018. Never had a problem paying my lot rent whether I was working my seasonal job or on unemployment in the winter. Found what I believed to be my dream job (2021) and hopefully my last job with a state pension and benefits and a decent starting wage of $17.00 per hour. Cooking staff was the only people that weren't union. They couldn't keep dishwashers for the life of them. When I started there where always 2 dishwashers on for Friday and Saturday for the restaurant I worked at to keep up with the volume. Couple months in we where down to 1 older dishwasher that I always helped while doing my pantry station. Then they started to messing around with the station's. Such as a line cook covering pantry and line or one of the other dishwashers covering pantry or having wait staff do some of it while I worked another food stand where I worked on Friday and Saturday. Monday I got hurt I was covering my station pantry and dish pit covering for our dishwashers day off. I don't mind helping somebody else while doing my own job. But to cover 2 stations while only getting paid for doing 1 is kind of ridiculous. And the fact that it was happening more and more often in the restaurant with other workers having to do the same thing. Sorry if that was long just needed to vent a bit.

I've never tried this before, but I'm pretty much at my wits' end. I currently have a New York State Workers' Compensation case going into permanency, and I'm also waiting on Social Security Disability for a hearing date on my claim. Haven't had any significant income in over a year. And I'm pretty much in desperation mode right now. This GoFundMe is just going to my past due lot rent on my owned mobile home, trying to possibly prevent my eviction (9/3/2025 was my served day) so I have 30 days from that date to come up with anything to hopefully satisfy my landlords of the trailer park.

Let me go into a little more detail on my current situation. I was hurt at work in 2021 October I received full workers compensation benefits until March 2022 when they reduced them. After they reduced them I was still getting by with no problems. On June 2024 my workers compensation benefits where stopped. Because of the reduction in 2022 I was partially disabled and showed of been attached to the work force by doing work searches and applying to jobs which was never said to me. It was then brought up in the winter of 2023. I obtained a new lawyer January or February of 2024 and was told to not do any work searches until the judge ruled on the case. I always received a call for hearing if I wasn't logged on to the virtual hearing, but never received the call nor did my lawyer so they ruled against me and stopped my payments.

I had a permancy doctor's exam in July 2024 it took over 1 full year from then to finally have a virtual hearing for my permancy and I gave testimony this Monday and it'll be another month to find out how everything worked out. Which is when I'm being evicted from my mobile home.

I've been working on getting a job currently, but with my permanent work restrictions are making it hard if not totally impossible. I've applied to quite a few in the last month. I've even gone to DSS for help as was told even if I got approved for emergency assistance since I have a 30 day eviction notice no money would be sent to my trailer park landlord. In addition, to this with DSS in June I went in for help with my electric being disconnected on the 12th. I was told they couldn't help me with it. Even though the New York State MyBenefits page states if you have a disconnect warning or have been disconnected on any of your utilities to contact your local DSS office for assistance.

I have this fundraiser set at $9,000 actually, but gofundme set it at 2,200 to start it out for some reason. My end still says the 9,000.

Every bit helps no matter the amount. I can thank everyone enough right now. Going to get a hold of my landlord tomorrow to ask what amount off of what I owe would delay the eviction 1 month maybe 2 to see the outcome of my workers compensation case around the time of my eviction date.
