Wheelchair Adaptation Needed

My name is Jana Thomas, I was born with Spina Bifida on Christmas Day, 1956.  I was able to walk with braces until about 10 years ago when my condition deteriorated.  I have never let my disability keep me from living and have made it my passion to help others by volunteering my entire adult life.  I have been a pet therapy volunteer for over 35 years, taking my dogs to hospitals, children's hospitals, nursing homes (the same one 27+ years!), schools, libraries and anywhere someone could use a little doggie loving.  I have also tested 100's of people with their dogs so they can become pet volunteers as well.  I also teach obedience classes (without pay) for a local dog club.
I live alone with my 4 dogs wonderful therapy dogs, I drive my own van and am very independent.
I'm asking for funding just for an expensive adaptation of a power wheelchair not covered by Medicare (they don't think it is necessary to help with daily living).  My only income is a very small monthly Social Security payment, that barely covers my bills.  Medicare has already authorized payment for the basic chair, so I'm only asking funding for the adaptation.
The adaptation is called an iLevel, it raises the chair so I would be able to reach things in my kitchen, when shopping, transfer more easily, safety in parking lots and relieve pain in my neck when looking up to talk to people.
This adaptation would make my life so much easier and allow me to help more people when I volunteer.   I have never asked for help before, always trying to manage on my own.  I lost my only family support 7 years ago when my Mother and brother died within 6 days of each other.  Sharing my dogs unconditional love keeps me going day by day.
