Hello, my name is Philip Pearson, I am 55 years old. I am currently on short term disability provided by my employer. I currently serve as a Safety Specialist in support of natural gas pipeline operations. Promoting a culture of safety is my passion, and I desire to continue serving in this profession. I am a veteran that served as an Army Aviator and Maintenance Test Pilot. I served as a pilot for a total of 21 years in both commercial and military operations.
 
I have diabetes and suffer from diabetic related neuropathy in my legs and retinopathy in both eyes. I will be going into cataracts surgery in both eyes to improve my vision; and surgery to amputate my right foot below knee. The good news is that through diet and medication, I have reduced my Hemoglobin A1C (HA1C) from over 12 to 6.4.  The bad news is, I am fiercely independent, single, and live alone. I have been blessed with support from family, friends and co-workers that have provided help when needed. I am now faced with lifestyle changes, and must face substantial expenditures to get a working prosthetic foot and make necessary modifications to my living arrangements (both home and vehicles).

 
Provided below are my objectives and the allocated cost for the essential needs to accommodate my disability.
