<div>Je m’appelle Nicolas Saavedra-Fusinato et je voulais prendre un moment pour vous parler d’une situation récente qui me touche de très près.

<div>Ma mère, Guendalina Fusinato, 64 ans, est hospitalisée depuis le 15 juillet 2025. Depuis quelque temps, elle avait les jambes enflées, en raison d’une rétention d’eau et d’une possible insuffisance cardiaque, en plus d’un diagnostic de neuropathie.

<div>Les épreuves médicales de ma mère ont commencé en 2006, lorsqu’on lui a diagnostiqué l’hidradénite suppurée, une maladie inflammatoire chronique qui provoque des abcès et des fistules finissant par s’ouvrir, couler et dégager des odeurs très difficiles à vivre.

<div>Cette maladie se diagnostique par des prélèvements qui, bien souvent, ne révèlent aucune pathologie identifiable. Sa cause reste inconnue, tout comme les facteurs irritants. Il n’existe pas de remède, et les traitements disponibles ne sont pas efficaces pour tout le monde : c’est de l’essai-erreur. Environ 4.1 % de la population mondiale est touchée, majoritairement des femmes.

<div>Ma mère a eu la chance de subir une opération aux aisselles et à l’aine. Les aisselles ont parfaitement guéri, sans rechute. L’aine a d’abord guéri, puis la maladie est revenue sur une autre partie de l’aine ainsi qu’au niveau des fesses.

<div>S’en est suivie une épopée d’une ampleur inimaginable : douleurs intenses, détresse psychologique sans fin et dégradation rapide de son état de santé. Pendant des années, elle a enchaîné les traitements, les consultations auprès de dermatologues et de chirurgiens, et des pansements de plus en plus volumineux, nécessitant l’aide quotidienne d’une infirmière du CLSC. Le système de santé l’a laissée tomber, et certains médecins, trop fiers pour admettre qu’ils étaient dépassés, ont refusé de le reconnaître.

<div>Elle a essuyé de nombreux refus et réponses décourageantes :
</div>
<div>— « Revenez après… »
</div>
<div>— « Ce n’est pas nécessaire pour le moment… »
</div>
<div>— « Si vous ne voulez pas participer à la recherche, je ne serai plus votre médecin… »
</div>
<div>Tant de rendez-vous dont elle ressortait en larmes, se sentant ignorée et sans aide.
</div>

<div>Il y a un peu plus d’un an, un abcès inhabituel a commencé à saigner. Ma mère a demandé à plusieurs reprises une biopsie à sa dermatologue, se souvenant qu’un résident lui avait un jour dit : « Si tu vois du sang, c’est un cancer. » La dermatologue a balayé cette inquiétude : « C’est un étudiant, il ne sait pas de quoi il parle. L’HS n’a rien à voir avec un cancer. »

<div>En octobre dernier, elle a été de nouveau hospitalisée pour diverses complications, notamment l’aggravation de sa maladie. Elle a demandé à voir l’équipe de dermatologie de l’hôpital, qui a refusé au motif qu’elle avait déjà une dermatologue attitrée. Ma mère a alors porté plainte à l’ombudsman. Le lendemain matin, l’équipe de dermatologie est finalement arrivée dans sa chambre, évoquant un “malentendu”, et a accepté de pratiquer une biopsie.

<div>Les résultats sont tombés : un carcinome épidermoïde (cancer de la peau), habituellement observé sur le visage ou les mains. Chez ma mère, il s’était développé aux fesses, dans la région anale, périnale, et vaginale, là où l’hidradénite avait déjà causé d’importants ravages.

<div>Elle a été prise en charge par l’oncologie, mais après une réaction au traitement d'immunothérapie utilisé, elle a été transférée en radio‑oncologie. Elle a reçu 20 séances de radiothérapie, qui ont aidé à assécher les lésions d’hidradénite et à éliminer le cancer dans la zone ciblée. La dermatologue a ensuite effectué deux autres biopsies, revenues elles aussi positives pour le même cancer. Le radio‑oncologue a indiqué ne plus pouvoir poursuivre : la dose maximale de radiation avait été atteinte.

<div>Puis, silence radio : aucune nouvelle, aucun traitement, aucune solution. Ma mère et moi avons commencé à pousser pour obtenir des réponses, à demander un plan, une option thérapeutique. Tous les médecins semblaient dépassés. Pour ma part — sans être médecin — j’ai trouvé de nombreuses études et pistes qui convergent vers une chirurgie radicale permettant de retirer le cancer et les zones atteintes par l’hidradénite. Une chirurgie qui aurait été bien plus simple il y a plusieurs années, avant le stade III et avant l’apparition du cancer. Après près de trois mois d’attente, on nous a finalement dit : « Ce n’est pas possible. » Non pas parce que l’intervention est irréalisable, mais parce que le chirurgien oncologue n’était pas confiant de pouvoir tout retirer : il n’avait vu qu’un seul cas similaire auparavant, qui s’était mal terminé, et il n’a pas de spécialisation dermatologique.

<div>Nous avons reçu une réponse d'un autre hôpital qui serait spécialisés en dermatologie-oncologie, et leur réponse est claire: au Québec, l'expertise nécéssaire pour son cas n'existe pas. Entre‑temps, ma mère a perdu l’usage de ses jambes, le personnel médical lui ayant demandé — et parfois même imposé — de rester alitée et de faire ses besoins "dans sa couche".

<div>De nouveau, c’est le bal des médecins qui défilent alors qu’elle est sous forte sédation. Quand j’ai constaté l’ampleur des erreurs et des incohérences, j’ai demandé un congé de travail pour pouvoir gérer ces bévues médicales et m’assurer que ma mère reçoive des soins de qualité.

<div>Dans l’attente d'une deuxième opinion à l'extérieur du Québec, nous avons eu raison d'être pessimistes quant à la capacité — et à la volonté — des médecins du Québec de tout mettre en œuvre pour l’aider à se rétablir, comme l’exige leur serment au sein de l’Ordre des médecins.

<div>J’écris aujourd’hui ces lignes depuis sa chambre d’hôpital, faisant une pause dans mes recherches, pour vous demander votre aide. Notre prochaine étape serait un transfert vers un centre spécialisé, à Toronto ou aux États‑Unis. Un tel transfert engendrerait des coûts énormes, bien au‑delà de mes moyens.

<div>My name is Nicolas Saavedra-Fusinato, and I wanted to take a moment to tell you about a recent situation that is very close to my heart.
</div>

</div>
<div>My mother, Guendalina Fusinato, 64 years old, has been hospitalized since July 15, 2025. For some time, she had been experiencing swollen legs due to water retention and a possible heart failure, on top of a diagnosis of neuropathy.
</div>

</div>
<div>My mother’s medical ordeal began in 2006, when she was diagnosed with hidradenitis suppurativa, a chronic inflammatory disease that causes abscesses and fistulas which eventually open, leak, and produce odors that are very difficult to live with.
</div>
<div><br />
</div>
<div>This illness is diagnosed through samples that, more often than not, reveal no identifiable pathology. Its cause remains unknown, as do the aggravating factors. There is no cure, and available treatments are not effective for everyone — it’s all trial and error. Around 4.1% of the world’s population is affected, mostly women.
</div>

</div>
<div>My mother was fortunate to undergo surgery on her armpits and groin. The armpits healed perfectly, with no recurrence. The groin initially healed as well, but the disease returned in another part of the groin and also affected the buttocks.
</div>

</div>
<div>What followed was an ordeal of unimaginable scale: intense pain, endless psychological distress, and a rapid deterioration of her health. For years, she cycled through treatments, consultations with dermatologists and surgeons, and increasingly large dressings, requiring daily assistance from a CLSC nurse. The healthcare system abandoned her, and some doctors — too proud to admit they were out of their depth — refused to acknowledge it.

<div>She faced numerous refusals and discouraging responses:
</div>
<div>— “Come back after…”
</div>
<div>— “It’s not necessary right now…”
</div>
<div>— “If you don’t want to participate in research, I will no longer be your doctor…”
</div>
<div>So many appointments from which she left in tears, feeling ignored and unsupported.
</div>

<div>A little over a year ago, an unusual abscess began to bleed. My mother repeatedly asked her dermatologist for a biopsy, remembering that a resident doctor had once told her: “If you see blood, it’s cancer.” The dermatologist brushed off this concern: “He’s just a student, he doesn’t know what he’s talking about. HS has nothing to do with cancer.”

<div>Last October, she was hospitalized again for various complications, including a worsening of her disease. She asked to see the hospital’s dermatology team, who refused on the grounds that she already had an assigned dermatologist. My mother then filed a complaint with the ombudsman. The next morning, the dermatology team finally came to her room, citing a “misunderstanding,” and agreed to perform a biopsy.

<div>The results came in: squamous cell carcinoma, a type of skin cancer usually seen on the face or hands. In my mother’s case, it had developed on her buttocks, in the anal, perineal, and vaginal regions — areas already severely damaged by hidradenitis.
</div>

<div>She was referred to oncology, but after an adverse reaction to the immunotherapy treatment used, she was transferred to radiation oncology. She underwent 20 sessions of radiotherapy, which helped dry out the hidradenitis lesions and eliminate the cancer in the targeted area. The dermatologist later performed two more biopsies, which also came back positive for the same cancer. The radiation oncologist said they could no longer proceed: the maximum radiation dose had been reached.

<div>Then came radio silence: no news, no treatment, no solution. My mother and I began pushing for answers, demanding a plan, a therapeutic option. All the doctors seemed overwhelmed. In my own research — though I am not a doctor — I found numerous studies pointing toward radical surgery to remove both the cancer and the areas affected by hidradenitis. Surgery that would have been far simpler years ago, before stage III and before the cancer appeared. After nearly three months of waiting, we were finally told: “It’s not possible.” Not because the surgery itself was impossible, but because the oncologic surgeon wasn’t confident they could remove it all: they had only seen one similar case before, which ended badly, and they did not have dermatological specialization.

<div>We received a response from another hospital specializing in dermatologic oncology, and their answer was clear: in Quebec, the necessary expertise for her case does not exist. In the meantime, my mother has lost the use of her legs, after being asked — and sometimes even forced — by medical staff to remain in bed and relieve herself “in her diaper.”
</div>

<div>Once again, it was a parade of doctors visiting while she was under heavy sedation. When I realized the extent of the errors and inconsistencies, I requested leave from work so I could manage these medical blunders and make sure my mother received quality care.
</div>
<div><br />
</div>
<div>While awaiting a second opinion outside Quebec, we have been proven right to be pessimistic about the capacity — and the willingness — of Quebec doctors to do everything in their power to help her recover, as their oath to the College of Physicians requires.
</div>
<div><br />
</div>
<div>I am writing these words today from her hospital room, taking a break from my research, to ask for your help. Our next step would be a transfer to a specialized center, in Toronto or the United States. Such a transfer would incur enormous costs, far beyond my means.
</div>
<div><br />
</div>
<div>If you can support us financially, you will be helping to give her back her autonomy and dignity — both of which she loses a little more each day due to the negligence of certain so-called “health professionals.”
