<div>Otoniel es un joven esposo y padre, conocido por su voz en la iglesia y por su servicio a la comunidad. Desde hace más de 4 años vive con asma severa y sinusitis crónica con pólipos nasales (CRSwNP). En ese tiempo fue sometido a tres cirugías de poliposis nasal. A pesar de tratamientos con esteroides, sprays y lavados nasales, la enfermedad ha persistido y afecta su respiración y su vida diaria.

<div>Qué ocurrió recientemente
</div>

</div>
<div>En junio de 2025, su condición se complicó gravemente al desarrollar una infección agresiva en los senos frontales que erosionó el hueso, diagnosticada como osteomielitis frontal causada por Klebsiella pneumoniae BLEE positiva (ESBL). Esto lo llevó a un cuadro séptico y a procedimientos quirúrgicos complejos para controlar la infección y retirar poliposis residual. Tras la cirugía principal, fue necesaria la reintervención en dos ocasiones por complicaciones posoperatorias (hemorragias/hematomas).

<div>Hoy está estable, en recuperación y bajo vigilancia estrecha.

<div>Equipo médico que lo atiende
</div>

</div>
<div>El caso está siendo acompañado de manera coordinada por:
</div>
<div>•	Infectología (manejo de la bacteria resistente y antibióticos).
</div>
<div>•	Otorrinolaringología (cirugías de pólipos y senos paranasales).
</div>
<div>•	Neurocirugía (procedimientos en la región frontal y complicaciones).
</div>
<div>•	Neumología (control del asma severa).
</div>
<div>•	Hematología (evaluación de sangrados y coagulación).
</div>

<div>Tratamiento actual y lo que falta
</div>
<div>•	Antibiótico intravenoso en curso: actualmente recibe meropenem. Gracias a Dios y a muchas manos solidarias, ya conseguimos el meropenem, por lo que esa parte del costo se ha reducido.
</div>
<div>•	Duración restante: el esquema antibiótico debe completarse entre 30 y 45 días (según evolución y cultivos).
</div>
<div>•	Después del antibiótico: si los estudios de control confirman la resolución de la infección, el paso siguiente será iniciar un tratamiento biológico especializado indicado por su otorrinolaringólogo para controlar el asma severa y la poliposis nasal recurrente. Este medicamento es muy costoso y se aplica de forma periódica durante meses.

<div>En qué se usarán los fondos
</div>
<div>1.	Atención hospitalaria: internamientos, emergencias y materiales quirúrgicos.
</div>
<div>2.	Honorarios profesionales: infectología, otorrinolaringología, neurocirugía, neumología y hematología.
</div>
<div>3.	Medicamentos y suministros: antibióticos IV (bombas, insumos, curas), analgésicos/antieméticos permitidos, profilaxis, etc.
</div>
<div>4.	Estudios y monitoreo: imágenes (TC/RM), laboratorios seriados y cultivos.
</div>
<div>5.	Tratamiento biológico posterior (dosis iniciales y controles) una vez se cierre el ciclo antibiótico con infección resuelta.
</div>
<div>6.	Cuidados de seguimiento: consultas, rehabilitación respiratoria y transporte médico cuando se requiera.
</div>

<div>•	Publicaremos actualizaciones semanales con el progreso clínico y comprobantes de gastos (mostrando solo lo necesario y protegiendo datos sensibles).

<div>Otoniel is a young husband and father, known for his singing at church and his service to the community. For over 4 years, he has lived with severe asthma and chronic rhinosinusitis with nasal polyps (CRSwNP). During this time, he underwent three nasal polyp surgeries. Despite steroids, sprays, and saline rinses, his condition persisted and has significantly affected his breathing and daily life.

<div>What Happened Recently

<div>In June 2025, his condition worsened significantly due to an aggressive frontal sinus infection that eroded bone, diagnosed as frontal osteomyelitis caused by ESBL-positive Klebsiella pneumoniae. This led to sepsis and complex surgical procedures to control the infection and remove residual polyposis. After the main surgery, two additional re-interventions were required due to postoperative complications (bleeding/hematomas).

<div>He is stable today, recovering under close medical supervision.

<div>Ongoing coordinated care involves:

<div>•	Infectious Diseases (resistant bacteria management and antibiotics).
</div>
<div>•	ENT/Otolaryngology (polyp and sinus surgeries).
</div>
<div>•	Neurosurgery (frontal procedures and complications).
</div>
<div>•	Pulmonology (severe asthma control).
</div>
<div>•	Hematology (evaluation of bleeding/hematoma and coagulation).

<div>Current Treatment and Next Steps
</div>
<div>•	IV antibiotic ongoing: he is currently receiving meropenem. Thanks to God and many generous people, meropenem has already been secured, so that portion of the cost is reduced.
</div>
<div>•	Time remaining: the IV antibiotic course must be completed over the next 30–45 days (depending on response and cultures).
</div>
<div>•	After antibiotics: if follow-up confirms infection resolution, the next step will be to start a specialized biologic therapy recommended by ENT to control severe asthma and recurrent nasal polyposis. This treatment is very costly and administered periodically for several months.

<div>How Funds Will Be Used
</div>

<div>2.	Professional fees: infectious diseases, ENT, neurosurgery, pulmonology, hematology.

<div>3.	Medications &amp; supplies: IV antibiotics (pumps, supplies, dressings), permitted analgesics/antiemetics, prophylaxis, etc.

<div>4.	Diagnostics &amp; monitoring: imaging (CT/MRI), labs, and cultures.
</div>
<div>5.	Subsequent biologic therapy (initial doses and follow-ups) once antibiotics are completed.
</div>
<div>6.	Follow-up care: clinic visits, respiratory rehab, and medical transportation when needed.
</div>
