<div>ich bin Tanja und ich und meine Familie sammeln für meinen lieben Mann Dieter, geb. in Ravensburg, Baden-Württemberg, Deutschland.
</div>

</div>
<div>Alles begann Mitte Mai 2023. Dieter verspürte plötzlich ein Kribbeln in seinen Fingerspitzen. Innerhalb einer Woche waren seine Extremitäten (Arme und Beine) komplett gelähmt.
</div>
<div><br />
</div>
<div>Das örtliche Krankenhaus, die Oberschwaben-Klinik in Ravensburg diagnostizierte das Guillain-Barre´-Syndrom (GBS), eine sehr seltene Erkrankung der Nerven.
</div>
<div>Sofort begann die Klinik die Standardbehandlung bei GBS, es wurden über 5 Tage lang Immunglobuline per Infusionen verabreicht.
</div>
<div>Doch die Lähmungen breiteten sich aus, die Atemmuskulatur wurde schwächer und auch die Sehnerven waren nun betroffen was sich in 'doppelbilder-sehen' auswirkte.
</div>

</div>
<div>Die Ärzte veranlassten nun eine sofortige Notarzt-Verlegung in die Universitäts- und Rehabilitationskliniken Ulm.
</div>
<div>Weil die Atemmuskulatur immer schwächer wurde und dadurch der Sauerstoffgehalt im Blut nicht mehr ausreichte, wurde Dieter nun intubiert und beatmet.
</div>
<div>Zwei Tage später wurde ein Luftröhrenschnitt gemacht, und eine Trachealkanüle zur Beatmung gesetzt.
</div>
<div>In Ulm wurde nun die zweite Standard-Therapie bei GBS versucht, die <a href="https://flexikon.doccheck.com/de/Plasmapherese" target="_blank" rel="noreferrer noopener">Plasmapherese</a> (Blutwäsche). Über 2 Wochen wurden Blutwäschen gemacht. Doch der Zustand von Dieter verschlechterte sich weiter.
</div>

<div>Eigentlich hätte nun klar sein müssen, daß es sich nicht um GBS handeln kann.
</div>
<div>Doch Dieter wurde nun (Mitte Juni) wieder per Notarzt-Verlegung in die Lungenfachklinik nach Wangen verlegt. Dieter bekam hier mehrere Infektionen die mit starken Antibiose-Infusionen behandelt werden mussten, auch eine Sepsis. Sein Zustand war sehr kritisch.

<div>Er lag auf dem Rücken und konnte außer seinem Kopf (hier ging nur Nicken oder Verneinen) nichts bewegen. Er war beatmet, hatte einen Katheter und viele Schläuche etc. pp, aber er war bei vollem Bewusstsein. Mir wäre lieber gewesen, er hätte von alle dem nichts mitbekommen, aber dennoch war ich froh, wenn ich ab und zu ein seltenes Lächeln von Ihm meinte erhaschen zu können.
</div>
<div>Kein Kratzen war ihm möglich, wenn es ihn gejuckt hat. Keine Fliege konnte er verscheuchen, wenn sie ihn belästigt hat. Mir war klar, daß ich in dieser schweren Zeit so oft wie möglich bei ihm sein wollte und für ihn da sein wollte.
</div>

<div>Nach weiteren 8 Wochen wurde Dieter, erneut per Notarzt-Verlegung, in das Therapie-Zentrum nach Burgau verlegt.
</div>
<div>Zunächst sagten mir die Ärzte Dieter bleibe ein 24-Std. Pflegefall mit Heimbeatmung. Das war natürlich ein Schock. Aber ich sagte wir kämpfen weiter und geben nicht auf.
</div>
<div>Die Ärtze forschten nun weiter, unter was Dieter leiden könnte (es war nun Mitte August).
</div>
<div>Es wurde nun das Blut weiter untersucht nach ganz seltenen Erkrankungen. Und Bingo, es gab Antikörper-Treffer im Blut.
</div>
<div>Die Diagnose lautete nun: <a href="https://www.sciencedirect.com/science/article/abs/pii/S1439484718300164" target="_blank" rel="noreferrer noopener">Paranodopathie</a> oder genauer: Akute autoimmune Polyneuropathie bei Paranodopathie.
</div>
<div>Eine äußerst seltene Autoimmunerkrankung, noch viel seltener als GBS. Gerade mal eine Handvoll Fälle sind derzeit in Deutschland bekannt.
</div>
<div><br />
</div>
<div>Nun hatten wir eine Diagnose aber der weitere Weg war und wird noch lang.
</div>
<div>Dieter bekam im Laufe des Aufenthaltes in Burgau eine Cortison-Stoßtherapie und wegen der Paranodopathie wurden ihm Rituximap-Infusionen gegeben. Diese legen das Immunsystem lahm und verhindern somit, daß das eigene Immunsystem die eigenen Nerven angreift.
</div>
<div>Dieter musste nun alles neu lernen, nach einer gewissen Zeit konnte er erste Finger leicht bewegen, dann die Hände.
</div>
<div>Außerdem wurde maschinell versucht seine Beine zu bewegen.
</div>
<div>Es war nun Mitte Oktober und Dieter wurde von der Beatmung entwöhnt, was nach so langer Beatmungszeit sehr schwierig ist, aber es gelang Dieter.
</div>
<div>Bis zu diesem Zeitpunkt war er nun nonstop 5 Monate beatmet auf Intensivstation.
</div>

</div>
<div>Ohne Beatmung konnte er nun langsam wieder essen, aber er musste noch gefüttert werden, da die Motorik in seinen Hände und Fingern noch nicht so funktionierte, daß er eine Gabel zum Mund führen hätte können.
</div>
<div>Im Laufe der Zeit konnte er dann auch die Zehen und dann die Beine leicht bewegen. Aber Belasten ging noch gar nicht. Nach fast 7 Monaten Liegen im Krankenhaus waren die Muskeln wie weggeschmolzen. Ständige Therapien verbesserten das nach und nach.
</div>
<div>Am 22. Dezember wurde Dieter im Rollstuhl nach Hause entlassen, um 2024 dann die weiteren langwierigen Rehas anzutreten.
</div>
<div>Er wurde mit dem Krankenwagen von Burgau nach Hause gefahren und wir konnten ein schönes Weihnachtsfest zusammen erleben.
</div>
<div>Ich kann nicht sagen, wie dankbar ich dafür bin.
</div>
<div>Doch der weite Weg wird noch lange. Wann Dieter wieder laufen kann wollten die Ärzte nicht sagen, es gibt zu wenige Fälle um da Vorhersagen zu machen.
</div>
<div>Die wenigen vergleichbaren Fälle verliefen unterschiedlich.
</div>
<div>Das Ziel von Dieter ist nun wieder laufen zu lernen, damit er eines Tages hoffentlich wieder mit mir zusammen in den Bergen wandern gehen kann.
</div>
<div>Wir wissen, daß das noch ein sehr langer Weg wird mit vielen langen Rehas, falls er überhaupt gelingt - aber Dieter kämpft jeden Tag dafür um aus dem Rollstuhl zu kommen.
</div>
<div>Dieter wünscht sich eine Verladehilfe für sein Auto um den schweren Elektrorollstuhl auch alleine verladen zu können.
</div>
<div>Das Bad muß umgebaut werden, und die Türstöcke sind zu schmal und die Ausstattung ungeeignet für Rollstuhlfahrer.
</div>
<div>Auch sind für seinen Genesungsprozess schon viele Kosten angefallen und werden auch noch anfallen.
</div>
<div>Deshalb haben wir als Familie uns zum Ziel gesetzt für Dieter zu sammeln.
</div>

<div>I'm Tanja and I and my family are collecting for my dear husband Dieter, born in Ravensburg, Baden-Württemberg, Deutschland.

<div>It all started in mid-May 2023. Dieter suddenly felt a tingling sensation in his fingertips. Within a week, his extremities (arms and legs) were completely paralyzed.
</div>
<div><br />
</div>
<div>The local hospital, the Oberschwaben-Klinik in Ravensburg, diagnosed Guillain-Barré syndrome (GBS), a very rare nerve disease.
</div>
<div><br />
</div>
<div>The clinic immediately began standard treatment for GBS, administering immunoglobulins per infusion over 5 days.
</div>
<div><br />
</div>
<div>But the paralysis spread, the respiratory muscles became weaker and the optic nerves were now also affected, which resulted in 'double vision'.
</div>
<div><br />
</div>
<div>The doctors now arranged for an immediate emergency transfer to the Ulm University and Rehabilitation Clinics.
</div>
<div>Because the respiratory muscles became increasingly weak and the oxygen content in the blood was no longer sufficient, Dieter was now intubated and ventilated.
</div>
<div><br />
</div>
<div>Two days later, a tracheostomy was performed and a tracheostomy tube was inserted for ventilation.
</div>
<div><br />
</div>
<div>The second standard therapy for GBS, plasmapheresis (blood washing), was now tried in Ulm. Blood washes were carried out over a period of 2 weeks. But Dieter's condition continued to deteriorate.
</div>
<div><br />
</div>
<div>Actually it should have been clear by now that it couldn't be GBS.
</div>
<div><br />
</div>
<div>But Dieter was now (mid-June) transferred again by emergency doctor to the pulmonary specialist clinic in Wangen. Dieter got several infections here that had to be treated with strong antibiotic infusions, including sepsis. His condition was very critical.
</div>
<div>He was lying on his back and couldn't move anything except his head (the only option was to nod or say no). He was on a ventilator, had a catheter and lots of tubes etc. pp, but he was fully conscious. I would have preferred if he hadn't noticed any of this, but I was still happy when I thought I could catch a rare smile from him every now and then.
</div>

<div>He couldn't scratch when he itched. He couldn't shoo away a fly when it bothered him. It was clear to me that I wanted to be with him as often as possible during this difficult time and that I wanted to be there for him.
</div>
<div><br />
</div>
<div>After another 8 weeks, Dieter was transferred to the therapy center in Burgau, again by emergency doctor.
</div>
<div><br />
</div>
<div>At first the doctors told me Dieter would stay for 24 hours. Nursing case with home ventilation. That was of course a shock. But I said we keep fighting and don't give up.
</div>
<div><br />
</div>
<div>The doctors continued to research what Dieter could be suffering from (it was now mid-August).
</div>
<div><br />
</div>
<div>The blood was now further examined for very rare diseases. And bingo, there were antibody hits in the blood.
</div>
<div><br />
</div>
<div>The diagnosis was now: paranodopathy or more precisely: acute autoimmune polyneuropathy in paranodopathy.
</div>
<div><br />
</div>
<div>An extremely rare autoimmune disease, even rarer than GBS. There are currently only a handful of cases known in Germany.
</div>
<div><br />
</div>
<div>Now we had a diagnosis but the road ahead was and will be long.
</div>
<div><br />
</div>
<div>During his stay in Burgau, Dieter received cortisone shock therapy and because of his paranodopathy, he was given Rituximap infusions. These paralyze the immune system and thus prevent your own immune system from attacking your own nerves.
</div>
<div><br />
</div>
<div>Dieter now had to learn everything again; after a certain time he was able to move his first fingers easily, then his hands.
</div>
<div><br />
</div>
<div>In addition, mechanical attempts were made to move his legs.
</div>
<div><br />
</div>
<div>It was now mid-October and Dieter was weaned off the ventilator, which is very difficult after such a long time on the ventilator, but Dieter managed it.
</div>
<div><br />
</div>
<div>Up to this point he had been on a ventilator in the intensive care unit for 5 months non-stop.
</div>
<div>Without ventilation, he was now slowly able to eat again, but he still had to be fed because the motor skills in his hands and fingers were not yet working enough for him to be able to bring a fork to his mouth.
</div>

</div>
<div>Over time he was able to move his toes and then his legs slightly. But it wasn't possible to put weight on it yet. After almost 7 months of lying in the hospital, the muscles seemed to have melted away. Constant therapy gradually improved this.
</div>

</div>
<div>On December 22nd, Dieter was discharged home in a wheelchair to begin further lengthy rehab in 2024.
</div>
<div><br />
</div>
<div>He was driven home from Burgau in the ambulance and we were able to have a wonderful Christmas together.
</div>
<div><br />
</div>
<div>I can't express how grateful I am for this.
</div>
<div><br />
</div>
<div>But the road will be long. The doctors didn't want to say when Dieter would be able to walk again; there are too few cases to make any predictions.
</div>
<div><br />
</div>
<div>The few comparable cases had different outcomes.
</div>
<div><br />
</div>
<div>Dieter's goal now is to learn to walk again so that one day he can hopefully go hiking with me in the mountains again.
</div>
<div><br />
</div>
<div>We know that it will still be a very long road with many long rehabs, if he succeeds at all - but Dieter fights every day to get out of the wheelchair.
</div>
<div><br />
</div>
<div>Dieter would like a loading aid for his car so that he can load the heavy electric wheelchair on his own.
</div>
<div>The bathroom needs to be remodeled and the door frames are too narrow and the equipment is unsuitable for wheelchair users.
</div>
<div>A lot of costs have already been incurred and will continue to be incurred for his recovery process.
</div>
