Medical Issues Causing Homelessness

I am completely embarrassed to be here now, asking for help, but I'm at a total loss as to what else I may be able to do. Please take the time to read my story, and donate if you can, but share even if you cannot. Thank you.
I have been suffering from a chronic medical problem for quite some time now. As a result of this condition I require a hysterectomy. I do have insurance which will cover this procedure, but I cannot be scheduled to have it for several months, and am unable to work until 4-6 weeks after having my uterus removed.... I cannot work in the meantime and have been out of work for more than a month now.
As a result of this illness I am behind on all my bills. I am past due on rent, as well as all of my utilities. I am in the process of filing for disability, but that is a very long process and may not see benefits for years. I have also attempted to go to Rowan Helping Ministries, but they do not allow help with rent unless there is provable income, which I no longer have. They may help with my past due power bill, and city bill (I'll know Friday at my appointment), but they will only cover what is past due, and I cannot pay for September.
I have come up with a solution that will help me through the next several months, however, this plan cannot be set in action until October, and I am living in fear of losing my home before then. I am a single mother of two little girls. My daughters and I have no place to go should we find outselves put out of the home I have worked so hard over these last several years to provide for us. I can cover all of my past due bills and September's bills with around $1500, with another $200 to cover gas, food, and other things that may come up.
This is my plea... Please help us. Everyone gets down on their luck, but this health issue could send my family to the homeless shelter. I'm a dedicated mother. I am a good person. And I help others whenever it is in MY power. So please, if you can help us now it would mean the world. Thank you for your time. <3
