Home Farm Family Medicine

Hi!  Home Farm Family Medicine is a new primary care practice in downtown Brattleboro, Vermont.  While we have a special focus on marginalized populations, including those identifying as LGBTQ+, people struggling with addiction, our homeless neighbors, and our home-bound neighbors,  mostly we are looking to provide good old-fashioned family medicine.  



The team so far:

Jeremy Morrison, DO - I am a board certified family physician who has been practicing in Windham County for  a little over three years now.  My special interests are child and adolescent medicine within the context of whole families, addiction medicine, transgender care, and aging in place.  

Moussou - Moussou is a Certified Medical Assistant who is helping both with visits and with building and curating the records system.  She and Jeremy have worked together previously in Putney, Vermont.  She also has a degree in Accounting from her birth country of Mali.  She has a special talent for making people feel welcome and part of our family.

Rebecca - Rebecca is managing the office as it develops.  She has many years of customer service experience and is currently working on another degree.  Our vision is to create a practice where people can get in touch easily and be seen or helped in the time they need or want to be. 

Joann - Joann 's work in our practice as RN Care Coordinator for our Spoke MAT program is funded by Vermont's Blueprint for Health.  She believes that everyone should be met at their level and treated with respect, and her approach to people really embodies this. 

Jake - Jake has taken on the mantle of Patient Navigator, having worked as a personal assistant and amanuensis for several years. He also has experience in IT and manages our network at HFFM.  He gets the most joy at work from helping people navigate complicated bureaucracies successfully.

Jane - Jane is a person with a checkered past, mostly in middle and high school education, but also (nothing proven in court) as a writer, filmmaker, and director.  She is both practical and creative, a good combination for a case manager.
