Help with end-stage liver disease

I'm Bill, entering middle age, and in the midst of battling various chronic medical issues that have now taken on an urgency that's led me here. A stroke in 2007 led to doctors discovering other issues that were eventually diagnosed as ulcerative colitis/pre-cancer in my colon - which was finally removed in late 2016, leaving me with a life that includes ostomy bags, among other procedures - along with primary sclerosing cholangitis and cirrhosis in my liver; an incurable liver disease that slowly develops over time and progressively gets worse, until a liver transplant becomes the only option. PSC also damages the bile ducts, which has started to affect my kidneys as well.

This is where I am. My eyes are constantly jaundiced, and I'm physically tired all of the time.

Even though I am surrounded by the best doctors and specialists you could imagine having, I'm without either family support or enough medical coverage - other than Medicare and disability from Social Security - to pay for the post-acute rehab, medication payments, and other things I'll need, in case I'm accepted for the transplant list. For people who receive liver transplants, the criteria are fairly involved - and proper post-surgery care is crucial in the decision. I believe there's a good facility here in Seattle, where I live, that I can receive that care from... and who also accepts Medicare - but, for a certain amount of time only. After that, expenses are my own to come up with. Having already survived a stroke, and having my large intestine removed.... I know that I have the will and desire to survive a transplant, and everything that follows it; but I need financial help. My symptoms are getting worse as the liver disease progresses,  and time is crowding in around me. The sooner I can present a good post-acute care plan to my doctors, the faster I'll be considered for the transplant list - and start the long process of getting through this. My next birthday is February 28th; if in that time I can get funding support, then I can be ready to go into the hospital.... when a liver becomes available - if not sooner.
