<div>My name is Wayne Wesley Johnson. I’m a multi-award-winning Professional Guitarist and Recording Artist! I’m 74 years old and still make my living performing in fine dining establishments, country clubs, concert, and wedding venues. I’ve been performing for over 6 decades.

<div><strong>I am a prostate cancer survivor (radiation/hormone therapy). Since 2018, I’ve been cancer-free for seven years, however, the side effects of those treatments have led to rather severe peripheral neuropathy and now, I need treatment for severe neuropathic pain not covered by Medicare insurance.</strong>

<div>I also need a complete right shoulder replacement! Shoulder pain is excruciating, 24/7. The shoulder surgery is scheduled for Oct 29, 2025.

<div>(Note: Last December, I had spinal fusion L2/L3, and bilateral laminectomies on L2, L3, L4, L5 &amp; S1). I’m still recovering from that, too!

<div>I am currently receiving Occupational and Physical Therapy in preparation for the shoulder surgery!

<div><strong>I will be unable to perform on my guitar and generate income to cover my basic living expenses, due to the inability to perform regularly and weekly, during the time of surgery and subsequent 3-month recovery period!</strong>

<div>My expenses are approximately $5,000/month! My wife passed away from cancer 1 1/2 years ago, so she can no longer help me, either!

<div><strong>I need financial assistance for both the neuropathy treatments as described in the attached letter from Total Health (approx. $8,000) and additional assistance for the lost income from being unable to perform weekly, for the three months following surgery (approx. $15,000).</strong>

<div>I am concerned about keeping my head above water during these challenging times! I don’t want to default on my living expenses, rent, etc., while I’m undergoing surgery, treatments, and recovery and unable to meet expenses.
