Some of you may know my 25-year-old daughter Lily and her decade-long journey to understand and seek treatment for her degenerative jaw disease characterized by progressive condylar erosion (the condyles are key parts of the jaw's temporomandibular joint that allow the jaw to open and close), leading to her mandible (lower jawbone) to gradually rotate downward and recede into her neck. On top of the significant psychological toll from the facial disfigurement caused by these degenerative changes, Lily became unable to fully open her mouth, chew hard foods, and participate in extended conversations. She also had difficulty brushing and flossing her teeth and suffered chronic, debilitating pain in her jaw, neck, and shoulders, as well as constant migraine headaches. Despite all these challenges, Lily has managed to persevere in her college education, on schedule to complete the last credit required for her to receive her BA degree from the University of Massachusetts in spring 2025 with a goal of next obtaining an MFA to pursue a career in fine arts.

<div>Like many others who suffer with "idiopathic condylar resorption" disease, Lily's medical journey has been long and arduous, full of dead ends with all variety of specialists. Thus, it was with great hope and not a little trepidation that on November 4, 2020, with her newly acquired Mass Health insurance in hand, Lily first met with renowned oral and maxillofacial surgeon Dr. Joseph McCain at the Massachusetts General Hospital in Boston and set out to undergo total jaw joint replacement surgery together with maxillary (upper jawbone) and mandibular osteotomies (surgical procedures that involve cutting and reshaping a bone) to hopefully restore her jaw function, mitigate her facial disfigurement, and relieve her chronic pain. After an initial "exploratory" surgery to determine the degree of inflammation ongoing in her jaw joints and more than a year of orthodontic procedures to pre-position her teeth for her anticipated new facial geometry, on December 4, 2023, Lily finally underwent an 8-hour surgery with Dr. McCain and Dr. Zachary Peacock (who did the osteotomies). Lily is grateful to Drs. McCain and Peacock for successfully arresting the disease process by introducing bilateral jaw joint prostheses and repositioning her maxilla and mandible such that she now has functional prosthetic jaw joints and a restored facial profile.

<div>Unfortunately, as Lily recently marked the 1-year anniversary of her Mass General surgery, she now suffers chronic swelling on the left side of her face and still unmitigated chronic debilitating jaw, neck, and shoulder pain with continuous headaches that impede her daily life. Symptomatic treatment with Botox injections, cranial nerve block injections, and anti-inflammatory medications have provided some temporary relief but are unsustainable and give rise to other health issues.

<div>Inexplicably and to our great dismay, Lily's surgeons at the Mass General are not open to treating her any further, insisting that the surgery was successful, seemingly insensitive to the pain and left-side facial swelling. Thus, Lily finds herself once again on her own to seek relief. After much research, Lily identified and consulted with another renowned oral and maxillofacial surgeon named Dr. <a href="https://drlarrywolford.com" target="_blank" rel="noreferrer noopener">Larry Wolford</a> at Baylor Medical Center in Dallas. During an in person consult, Dr. Wolford was able to identify the likely source of Lily’s chronic pain [a leftward shift of the mandible and maxilla causing impingement of her atypically enlarged left coronoid process (a thin, triangular bony projection of the mandible that plays a vital role in chewing and stabilizing the jaw) against her left zygomatic arch (cheek bone) and a small overbite preventing her molars from making contact] and made the following treatment recommendations:

<div>We were overjoyed to receive recommendations finally identifying the source of her pain and detailing an action plan to address it. However, MassHealth is not willing to provide financial support for these procedures, as Dr. Wolford is out of network, and the cost is far beyond our means. I have managed to enroll Lily (retrospective to 11/1/24) onto my work Aetna insurance plan but am told that best case scenario she might get 10% of the surgical (and none of the orthodontic) cost covered this way. Not including airfare, lodging, and ground transportation for the initial surgery and for three subsequent follow up visits, Lily is told the cost of the surgery (including the follow ups) and the orthodontia will be, respectively, approximately 70,000 and 9,000 USD. Which brings me finally to the main point of this campaign where I ask that you consider whether you might help Lily with the cost of this essential surgery so that she can hopefully move forward with her young life unimpeded by chronic pain.
