I, Ralph Michael Monsalve Pertsinidis, of legal age, identity card No. 18.972.487, residence in Barinitas, Barinas, Venezuela.
I greet you very respectfully and I want to tell you about my health conditions and tell you that because of them I am unable to continue with the exercise of my role as a cyclist. Cycling has been my dream all my life and I have represented my country, Venezuela, in many other countries, but it must be said that cycling is a sport of great sacrifice. I have been presenting recurrent pain in my left leg for almost 1 year, which has made practically impossible to continue cycling because it makes pedaling movement impossible for me and has reached the point of almost making it impossible to walk. A Dr. Angiologist examined me to perform a Duplex Doppler scan of the lower limbs, which yielded the following results:
__ Right lower limb Minimal peripheral obstructive arteriopathy, parietal calcification and intimal widening.
__ Left lower limb: predominantly iliac femoral muitisegmentary peripheral obstructive artery disease with severe hemodynamic repercussions.
For this reason, the Dr. recommends the realization of a study called Angiography of the Aorta and lower limbs, in addition to the cardiology evaluation to perform an arrhythmia Holter, TTE; all this with the purpose of determining the procedure to follow for the eradication of said arterial obstruction that is harming me considerably. A possible hypothesis of this may be a catheterization, but the means cannot be specified until the results of said studies are obtained.
In addition to that, I present a condition in my spine: deformity in the L4, LS, S1 discs, which are pressing on the sciatic nerve, affecting, together with the arterial obstruction, the normal functioning of my left leg. According to the consideration of the Spine Traumatologist, Dr. Marlon González, I must undergo a surgical procedure to correct this problem.
By virtue of the fact that I belong to a non-profit cycling foundation, I do not have the necessary resources to be able to undergo such medical procedures, therefore I see the urgent need to ask for your valuable collaboration in order to continue with the medical recommendations for my case.
I have to undergo very expensive studies and procedures and in Venezuela it is almost impossible to get the money to make them. For that reason I take this way to ask your help and the grace of God Almighty. The studies and procedures that I must carry out are:
__Angio Tac, Angioplasty on the left leg and Spine, for an approximate value of: $ 340, $ 2800, $ 1860, respectively. For an approximate Total of $ 5000.

Los saludo muy respetuosamente y deseo contarles sobre mis condiciones de salud y decirles que por las mismas me encuentro imposibilitado para continuar con el ejercicio de mi función como ciclista. El ciclismo ha sido mi sueño toda la vida y he representado a mi pais, Venezuela en muchas paises, pero debe decir que el ciclismo es un deporte de mucho sacrificio. He estado presentando recurrentes dolores en mi pierna izquierda, desde hace casi 1 año, lo que ha hecho practicamente imposible continuar haciendo ciclismo pues me imposibilita el movimiento de pedaleo y ha llegado al punto de casi imposibilitar caminar. Me examinó un Dr. Angiologo para realizar una exploración Doppler Duplex de miembros inferiores, lo cual arrojó las siguientes resultados:
__ Miembro inferior derecho Arteriopatia obstructiva periférica minima, calcificación parietal y ensanchamiento intimal.
__ Miembro inferior izquierdo: Arteriopatia obstructiva periférica multisegmentaria predominantemente iliaco femoral con repercusión hemodinámica severa.
Por esto, el Dr. recomienda la realización de un estudio denominado Angio tomografia de Aorta y de miembros inferiores, adicional a la evaluación por cardiologia para realizar un Holter de arritmia, ETT; todo con la finalidad de determinar el procedimiento a seguir para la erradicación de dicha obstrucción arterial que me está perjudicando considerablemente, una posible hipótesis de esto puede ser un Cateterismo, pero no se puede especificar el medio hasta no tener los resultados de dichos estudios.
Aunado a eso, presento una condición en mi columna vertebral: deformidad en los discos L4, LS, S1, los cuales están presionando el nervio ciático, afectando en conjunto con la obstrucción arterial, el funcionamiento normal de mi pierna izquierda. Según consideración del Traumatólogo especialista en columna, Dr. Marlon González, debo someterme a un procedimiento quirúrgico para corregir dicho problema.
En virtud a que pertenezco a una fundación de ciclismo sin fines de lucro, no cuento con los recursos necesarios para poder someterme a dichos procedimientos médicos, por ende me veo en la imperiosa necesidad de pedir su valiosa colaboración para poder continuar con las recomendaciones médicas para mi caso.
Debo someterme a estudios y procedimientos muy costosos y en Venezuela es casi imposible conseguir e
el dinero para hacermelos. Por esa, razon recurro a esta via para esperar su ayuda y la gracia de Dios, Todopoderoso. Los estudios y procedimientos que debo realizarme son: Angio Tac, Angioplastia y Columna, por un valor aproximado de: $ 340, $2800, $1860, respectivamente. Para un Total aproximado de $5000. Agradezco de antemano cualquier ayuda que puedan darme.
