HEALTH LIFELINE Inc. is a 501c3 non-profit charity organization, staffed entirely by non-salaried volunteers, composed of physicians, nurses, teachers, librarians, and business people who give most generously of their time, energy, and skills. A secular, non-religious entity, we are located in a convent building space donated rent free by St Ann’s Church in Nyack. Our Board of Directors is composed of civic-minded citizens and officials in Rockland County.
We provide follow-up care to indigent patients that have been discharged from Nyack and Good Samaritan Hospitals in their Emergency Rooms. We also work closely in coordination with Rockland County Department of Health, Hospice of Rockland, and many other service organizations in Rockland County. We are open two days weekly, and monitor phones on off days.
Our patients are mostly immigrants, poor, and ineligible for Medicaid or any health insurance, or cannot afford the insurance co-pay. These patients have no where to turn for ongoing health care, except the Hospital Emergency Rooms. This is a waste of ER resources because these patients need continuing care of chronic conditions such as high blood pressure, diabetes, heart disease, arthritis, thyroid problems, occasional infectious diseases, women's healthcare and PAP smears. By keeping our patients out of the hospitals, we are saving an estimated $200,000 public dollars per year.
Dr. Hamilton is assisted by senior colleagues with sub-specialty training in Surgery, Cardiology, Oncology, Dermatology, Hematology, Endocrinology, Gynecology, etc., who advise him on complicated problems.
Health Lifeline is totally dependent on donations, grants and contributions for funding of our operation. All funds are dedicated to providing patient care. We have no salaried employees or paid fund-raisers.
Our annual program cost is $24,000. Your donation will help us reach our goal this year. Your gift is entirely tax deductible .
