THE HOME TEAM

HOME & COMMUNITY BASED SERVICES

SERVE PEOPLE IN THEIR HOME ENVIRONMENTS OR AT COMMUNITY LOCATIONS OF EACH INDIVDUALS PREFERENCE.

HOME HEALTH

Client services which require skilled nursing support. Typically, a Registered Nurse (RN) assessment, and implementation of a plan of care, to include interventions based on a primary care provider (PCP) orders. Such as, specialty wound care, administration and management of medication(s), lab/blood collection, health monitoring, etc., as needed.

HOME CARE

Under the supervision of the RN the trained caregivers, typically Certified Nursing Assistants (CNAs) provide support with basic activities and functions also known as, activities of daily living (ADL) and/or instrumental activities of daily living (IADL). Examples are personal care, light housekeeping, some exercising, assisting with ambulation, assistance with errands, etc., as needed.

Living on Lāna’i, Hawaii - we do not have a lot of options for our Kupuna. Most of them don’t want to be in a “facility”, or can’t even afford for to be in one.

Why make them feel even more uncomfortable?

When all they want to be is at “Home”.

Our mission is to provide compassionate care to our Kupuna in their own home environment.

We are asking for assistance with funding for these services that is desperately needed in our community.
