Annalise's Kidney/Pancreas transplant

A few of you may know a little bit about my situation. And I need to fill the rest of you in because
I am asking for your help.
I have been a Type 1 diabetic for the last 17 years. It has been a roller coaster of issues the entire time. Over the years it has taken a tremendous toll on me both physically and emotionally, not to mention financially.
For one thing, my eyesight has really been affected. I have already had two cataract surgeries as well as several laser procedures. My eyesight is ok at the moment (not great) but I may face future procedures and will never have the vision I once used to have.
One blessing that the Lord has given to me was the birth of my daughter. To become a mom through all of this was amazing. My daughter is the best thing to ever happen to me. Raising her on my own has been tough however. Sometimes it breaks my heart that she is sometimes forced into the position of caretaker when my bad days get the best of me. She has stepped up to a plate that many people never have to face in a lifetime. She has had to endure a lot in her short 12 years.
Unfortunately, life is throwing another obstacle in my way. On November 1st, 2012, I was diagnosed with ESRD (End Stage Renal Disease). Basically that means that I have Stage 5 kidney failure. My kidneys failed suddenly and I had to start dialysis on the spot. At my age, being on dialysis and being a Type 1 Diabetic, I desperately need a kidney and pancreas transplant. My doctors think this is my best chance. My best chance at getting this double organ transplant is for me to relocate. I need to transfer dialysis centers and get registered with a transplant center as soon as possible. I can then get on the transplant list. But that can only happen if I have finances in place. They will not proceed without me having financial security. I do have Medicare to cover a portion of the transplant, but that won't be nearly enough. I need to be able to cover certain out of pocket expenses included with relocation, transportation, living expenses, work up testing, transplant costs, anti-rejection drugs, follow up monitoring and a plethora of other expenses.
My greatest fear is that I am a burden to my family and that I wont be around to see my daughter grow up. We have already had a few scares but I have made it this far. I know now that the kidney failure is making it that much harder on my body to hold on.
I would like to ask all of you to look inside your hearts and help me. I am thanking you in advance for helping me get one step closer to a transplant so I can have another chance to live a better quality of life to be around to see my daughter grow up.
