Dr. Angel White & Shadae Booker-Shelton are board-certified Family Nurse Practitioners. We specialize in primary care and care for people of all ages. We are the owners of A & S Divine Healthcare. Over the past 2 years we have been working diligently to create and provide healthcare services to the underserved populations. A & S Divine Healthcare mission is to provide high-quality healthcare services to the communities we serve. Our goal is to transform the way healthcare is delivered by eliminating barriers to access, reducing health disparities, and decreasing healthcare costs. We will be offering services for:

·         Health promotion and disease prevention
·         Primary Care/Management of chronic conditions
·         Treatment of acute illnesses
·         Physicals (work, school, sports, DOT)
·         In-office testing (Strep, Flu, Covid, STI, etc.)
·         Obesity counseling & weight management
·         IV hydration & Vitamin therapy
·         Telemedicine

We have secured a unit for our healthcare clinic in the south suburban area of Illinois. We are seeking to raise supplemental funds to fully renovate the clinic, purchase equipment, and supplies. We are asking for your support to help us bring our vision to life which is to exceed the needs and expectations of those we serve to improve the health of the people. Our goal is to raise $30,000.
