Funeral Expenses

Who was Jon Michael Forman?  He was the oldest of 4 children, a husband and a father.  After graduating from high school, Jon volunteered to serve our country with the United States Marine Corps and is a Gulf War Veteran.  While in service Jon was promoted several times, he received 11 medals and retired as a Corporal within the USMC-2d Battalion, 5th Marines, 1st Marine Division. 

After retiring from the military Jon worked as a physical therapist. His job was to help injured and ill people improve their movement and manage pain.  Jon joined the profession to help others.

In 2005, Jon along with his wife Karen, became the caretakers to his father, who was diagnosed with dementia.

On August 5, 2019, Jon Michael Forman was diagnosed with Stage 4 Lung Cancer at the age of 48.  Shortly after being diagnosed, he lost the battle while surrounded by his wife, sons, daughter and father on Tuesday August 27, 2019. 

While Jon would go out of his way to help friends, family and neighbors, Jon did not have any life insurance.  The family did not know about this until Jon's passing.

The money raised with this GoFundMe account will go to pay his funeral service expenses.  Any additional dollars raised will go to help with the following:

A trust fund for his 14-year old daughter Megan who just started her Freshmen year of high school when Jon was diagnosed. 

In addition, remaining money will be used to help his wife Karen during the transition to help with the mortgage and medical expenses for the in-home care for her father-in-law.
