Medical and financial assistance

My name is Jose Monteleon and recently I was diagnosed with  a pulmonary embolism and also dealing with issues with my liver, heart and lungs. A couple months ago I started dealing with chest pains and shortness of breath and went to the ER and was sent home. Shortly after that I collapsed and had to be rushed to ER in ambulance. Doctors then discovered the first blood clot in my lung. Since then pain and shortness of breath became more severe and more frequent. We ran some tests and my heart isn’t working properly when under stress which is putting me at an increased risk of a major cardiac event. So with all of this going on I have been unable to work and am not sure when I will be able to return to work. I’ve decided to swallow my pride and ask for help until I am able to return to work.
