Desperate: help with rent and health insurance

UPDATED!! URGENT!! I have not been paid my July 2013 unemployment payment at ALL, so I have been unable to pay my July rent or pay for food at all! :( I know people helped me out a lot last year, but I am again in desperate straights as I don't know how I can put off paying my rent much longer (I have emailed my landlord the situation and I'm waiting to hear back)! I also don't have food money. Please please, if you can help at all, please donate so that I can even make a partial rent payment! I have changed my fundraising goal to reflect the $650 rent and food money and the $$ that GoFundMe takes out of donations, but if you want my address to mail a check instead, please let me know. Thank you!

I had someone who had been willing to pay for my September health insurance premium (I pay for it, not my employer) which is due THIS THURSDAY, but that person has fallen on dire straits of their own, so they CANNOT help me AT ALL.
This means that I will have to use $420 of the $$ already raised, to pay the end-of-August health insurance premium, leaving me AGAIN without my September rent (due a few days later, Sept 5th).
I was told TODAY by the hospital that I will probably be off work (they will NOT ALLOW me to return to work) for all of September! :( That means I will not have October rent AT ALL.
So, I am hoping to raise this higher $$ amount ($420 health insurance premium $430 rent $630 October = $1480 total PLUS GoFundMe takes 5%, so I actually need $1554. Yikes.).
I am so sorry to be asking for further assistance! :( I am embarrassed to be this poor, but I don't know where else to turn. I have no other avenue.

As of August 6th, I have been placed on disability leave from my job and have NO INCOME, due to having to have treatments at the hospital that last most of the day, every day. I have applied for CA state disability insurance, which (when it comes in a month, estimated) will probably cover my health insurance, my car insurance (to get back and forth to the hospital every day), my phone, and a small amount for food. It will not even begin to make a dent in my rent, and again, the disability insurance is expected to take another MONTH before they start paying me.
I don't know when the doctor's will allow me to return to work. I hope and pray soon, but for now, I am worrying only about September's rent.
I have applied to my church and to another charity, and have raised $200 of my $630 rent by those sources. I need another $430 by September 5th (last day before my landlord has to pay his mortgage) or I face being homeless.
Please, if you can help at all with my September rent, I would be eternally grateful. I would be happy to answer more questions either one-on-one or here on this page. I can provide proof I'm in the hospital every day, and that I've applied for disability (but I can't provide proof of how much they'll eventually pay me, I don't have that letter yet).
I am perilously close to losing my 3-years-stable living situation, and I don't know what else to do besides ask for help. Please, know I would not ask were I not at my wit's end. Please consider helping any bit you can.
