Help Woope and her family with Home

My name is Sydney Keith Claymore my daughter is Woope Claymore. We are from Cheyenne River Sioux Indian reservation, and live in Rapid City South Dakota.
My daughter Woope  was diagnosed with Systemic Lupus Erythemotosis when she was 17. She was granted a Wish from Make-a-Wish. She is walking in the Above photo to raise awareness and donations for the Foundation, on one of her good days.
 Woope suffers what only 5% of all Lupus patients suffer. She has had congestive heart failures, The Lupus took out her Kidney, Then attacked her Brain and she suffered Seizures due to 6 lesions on her Brain it attacked the lungs, her liver, she had pancreatsis, Then she received the Kidney Transplant April 1, 2003. After a year she was back to normal for 4years, then in 2008 she started having health problems. The Lupus came out of remission and attacked the Transplanted kidney. Woope went back on Dialysis October of 2009. She suffered through Congestive heart failures, Pluresy, Blood infections. Losing her gall bladder. Diagnosed with Gastroperesis, unable to eat normally. Had a slight heart attack and had two stints put in. Surgery after surgery to try and keep her on Home Dialysis but she had to go on Hemo-Dialysis in 2014.  Now on Febuary 14th 2017, she was told she now has to do battle with Cancer. She weighs only 80lbs. now. 
We live in seperate homes but because she is always getting sick and I need to be by her 24/7 to help take care of her, we are in need of a home and a vehicle to drive her to and from Dialysis 3 times a week.
Our situation is Urgent. We need a home right away. Our goal of $5000 would help us to get a used vehicle and a home together immediately. Woope wants to live, and wants to be healthier, she wants to be with her loved ones, to enjoy the company and to pray together. Also we would appreciate any assistance from potential Landlords. If anyone has a house to rent to a close knit family.
