At Risk of Eviction. Please Help!

Please, we really need your help!
Hi everyone and thank you SO much for looking at my page here.
My partner and I are not able to meet our rent this month, nor will we be able to next month. We cannot afford all the food, medicine and household expenses our family needs. Though we've been managing to stretch those dollars, they can only stretch so far.
This is a temporary situation.
In a nutshell: I am disabled and receive Federal Disability. I receive Personal Care Assitance from the State. My partner provides that care. At my last assessment, despite several things having gotten significantly worse, my hours were cut from 4 3/4 to 1 1/4 hours per day. I have been fighting it. My appeals were denied and I have now asked for a new assesment, armed with letters from doctors and our own documentation. But, like everythng in the government it is a slow process.
In the meantime my partner's income has been slashed by 75%.
We have managed, barely, until now, but we simply can't do it this month or next. By then, if all goes right, the new assesment will give us the hours back and we'll be able to meet our expenses again.
Neither of us have family we can go to for help, and if we do get evicted....we have no place to go. None of our friends has room to fit us (even on couches).
The money we are asking for breaks down to 1 months rent ($905), late fees for not being able to pay this month's full amount in time ($40), and some money toward food, meds, and household necessities ($300).
