Need a big helping hand

My family and I are going to be homeless as of Monday. I've never been the one to ask for help in previous years we've always picked a family and helped them out at holidays. I've been having health problems for almost a year now. I've been in and out of emergency rooms , I was in patient for a week and all they came up with was a severe vitamin D deficientcy. They are trying to figure out what's 6. I just can't keep up financially. We lost our house a year ago and now about a week ago I had to have emergency surgery on my spinal cord and had a spinal fusion.(they removed c5 and fused c4&6 )That would solve the questions of why I've been falling down, I'm confused at times ,my memory is bad, my speech is slurred and many other things. WOW is this painful !! I'm not able to do much and I certainly can't help my wife, (of 28 years) move our things to storage. So she was trying to move things to a storage unit . When my tool chest rolled down the ramp and into her kn awwee. Now she's in a knee brace and we don't know to what extent her injury is yet. I also have a special needs adult son who lives with us and he is disabled . And of course our lovable dog lives here to. I already needed to get surgery on my back, and a hip replacement too but, those will take even longer at this rate. I am going to therapy for 3 hours twice a week and with the many other appointments, and other places we need to go, we are teetering on a very short line with our car that was was made in the nineties. It already has many problems and we've already had to put a lot of money to keep it afloat. Plus right now I have to pay aprox. $600 a month to keep my much needed health insurance. We really need help in all aspects, it feels like life is drowning me. I never thought I would fall apart at such a young age, I am only 52. If there is anyone who is able to help me out, that would mean more to me than anything, even football!! ( Lol) But seriously, friends near and far, we desperately need some help and I don't know how long my recovery is going to take. We need a place to live, I can't imagine sleeping in my car in my or my family's condition. We continue to need help, I've re injured my neck several times or it's messed up from the surgery? I don't know, but I do know I've got way too much pain! My memory is not fixed nor is my spacial difficulties. This is embarrassing!(!!! I WANT MY LIFE BACK!!! It's too cold here in Michigan to be in a car!!! And my car has no heat, a broken trunk latch so I have to use a bungee cord and my windows for up and down only when they want to, I swear!
