Life Saving Mayo Clinic Appointment

I NEED TO GET TO THE MAYO CLINIC ON THE 23rd. I moved to Tennessee in 2017 and have been battling unexplained medical conditions since November 2017. I have had 3 massive Saddle pulmonary embolism, A bone spur removal in September 2018 which I fell on and ruptured my Achilles ‘s tendon. Which one of those pulmonary embolisms happened after that so Achilles tendon repair did not happen until September 26 of 2018. So, the back of my heel is worse and keeps causing open holes. Due to my blood clots and the massive amounts of blood thinners surgery is not possible.I have also been diagnosed with multiple sclerosis and dystonia. I am now currently suffering with 8 blood clot in my right leg and still have 2 blood clots in my lungs. This is with 2 IVC filters that are still letting them pass to my lungs. My doctors have told me that I am in a life and death situation and my only answer is the Mayo Clinic in Minnesota. I am in desperate need of assistance to be able to go. You help would be greatly appreciated.
