medical, house, and transport troubles

I live with my grand parents and five other family members in one house and 3 are disabled and grandfather is a veteran both grandparents have been denied disability 2-3 times and my grandmothers health insurance just went up to over 1k a month and thats hers alone my grandfathers is 700$ and we also have a extremely large house bills that are constantly racking up (power, water Etc.) and me and my mom are the only ones with a job and I just recently had a little boy (a few weeks old) and mine and my fiance and son's only form of transportation is starting to go bad transmission and motor wise has Balding by the day tires. Me and my mom are only people with a job and we don't make hardly enough to make it by and this our first outreach for help cause at times its embarrassing to ask for help but we can no longer go without trying. every little bit will go twoards bills for my grandparents and next few months house bills and tires.
