Personal Funds for Healthcare

Hello, my fiance and I are in a struggle. We have been strongly working towards finding a job, but we just aren't having the luck we'd hoped for. We were both declared eligible for Medicaid, however due to a small mishap, my whole amount of government assistance (food stamps and potential Medicaid) will be closed on August 11th, unless I can get a form filled out and mailed in properly before that date. 

If our benefits are closed (food stamps and potential Medicaid), I really don't know what we will do. We do not have a single dollar to our name and I am in debt. We both have back problems, constant headaches, and I have recently come to a realization that I may have high blood pressure and anxiety, for which both can get bad if untreated. I also know that I'm easily susceptible to yeast and urinary tract infections. My fiance and I both have issues that we would like to get answers to, or even address to a doctor.
